Common IURTC Forms
Technology Commercialization
Inventions come in many forms, from software and devices to research reagents, new medications, and new uses for existing products. For each, the process is similar. Download the appropriate forms (available in Microsoft Word format) and submit them to an IURTC technology manager.
Pre-Disclosure
Not sure if your idea is ready for protecting and marketing? Contact us or complete a Pre-Disclosure Form.
Types of Discoveries
If your discovery is an invention, complete an Invention Disclosure.
If your discovery is a biological research tool such as an antibody or a mouse model, complete a Biological Material Disclosure.
If your discovery is a literary work, visual art work, performing art work, sound recording, serial or periodical, or mask work (diagrams of integrated circuits), complete a Copyright Disclosure.
If your discovery involves computer software, you will need to complete a Software Disclosure Form.
Additional Inventors/Authors
If your discovery includes more than one inventor/author, each additional inventor will need to complete an Additional Inventor/Author Form.
Revenue Sharing
If you and your co-creators plan to share licensing revenue in unequal proportions, complete a Revenue Sharing Form.
